Depuis de nombreuses années, le nombre de prises en charge de personnes âgées est en augmentation au sein des structures de soins belges. Ce phénomène est également ressenti au sein des Cliniques Universitaires Saint-Luc. Et ce, autant au sein des soins d’urgences que des unités de soins.
les prises en charges se voient généralement perturbées par la complexité des situations rencontrées. De nombreux praticiens issus de différentes disciplines et spécialités se voient mobilisés autour du patient et de son entourage. Les voies d’entrées dans le système de soins sont assez différenciées : médecin généraliste, consultation de spécialiste, « one-day », hospitalisation, admission aux urgences. Plusieurs spécialités sont directement concernées par ces prises en charge. Parmi celles-ci, on distingue entre autres, la gériatrie, la neurologie, la psychiatrie. Nous faisons le choix de réaliser notre recherche au sein de ces unités de soins, bien que d’autres (orthopédie, cardiologie, etc.) pourraient également être étudiées.
Selon l’envoyeur, le mode d’admission ou l’histoire de la personne, l’accompagnement du patient psychogériatrique pourrait être plus ou moins variable.
ce mémoire tentera de clarifier le rôle et l’importance des structures mobilisées autour de cette problématique et de ce type de patientèle, au sein des Cliniques. Nous tenterons d’évaluer le nombre de patients psychogériatriques parmi les unités principalement concernées et d’isoler quelques données afin de tracer un contour assez précis de l’identité de ces patients aux Cliniques Universitaires Saint-Luc.
Dans le but d’offrir une approche plus précise de la question, il conviendra de redéfinir le profil du patient psychogériatrique ainsi que les partenaires concernés par sa prise en charge.
Par la suite, nous tenterons d’établir le processus de prise en charge existant au sein des Cliniques et d’ne définir les limites et l’activité, sous forme de flux en processus.
Enfin, après analyse de ce processus et d’expériences réalisées au sein d’autres structures, nous tenterons de tracer les contours d’un circuit de soins spécifique à la prise en charge de ce type de patients aux Cliniques.
Ces dernières années, ce sujet de travail a déjà inspiré plusieurs étudiants de l’école de Santé Publique. Nous tenterons d’articuler ce travail dans la lignée de celui réalisé par I. Pauls qui tentait de développer les deux première étapes de la démarche de planification selon D’Hoore et Tonglet

We are often called in rest homes to handle the behavioral problems of our aging patients. We often feel a kind of discrepancy between the situation, as described by the nurses, and our clinical pattern. A better understanding of the main processes, used on the caregiver's request, is crucial to obtain an optimal treatment. A survey has been proposed to four geriatric nursing homes in Brussels. The main goal was to show which behavior troubles are considered by the nursing team as causing the most difficulties in their daily practice. The conclusion of this paper is that a multi-disciplinary approach is needed to handle those specific problems. The general practionner could be the hub if this approach. Nous sommes souvent appelés en home pour gérer les troubles du comportement de nos patients vieillissants. Il existe souvent un décalage entre la situation décrite par les soignants et nos impressions cliniques.La compréhension des processus mis en œuvre dans l'expression de la demande des infirmiers et des aides soignants est capitale pour proposer un traitement optimal. Un sondage a été proposé à quatre maisons de repos de Bruxelles. Il a pour but de mieux percevoir quels troubles du comportement posent d'avantages problèmes aux équipes soignantes. La conclusion de ce travail est qu'une approche pluridisciplinaire est nécessaire à la gestion de ces situations particulières. Le médecin généraliste pourrait en être la plaque tournante.
